I've had a lot of success using alt-n's MDaeomon (www.altn.com). Works
great. I also use GroupMail for email campaigns. They are not CF specific,
but again, email should be left to mail servers, not web apps (in my
opinion).

output the completed PDF as a PDF and insert the form values into a database

If you want to dabble in some Java you can use the iText library to write
the captured data to the pdf form and then push the resulting pdf file to
